Vision Care Registration Policy
To ensure we provide you with the best possible care, all patients must complete our Vision Care Registration Process before receiving any consultations, diagnostic tests, or treatments.
1. Registration Requirements
All patients must complete a Patient Registration Form either online or in person at reception.
You will be required to provide:
Full legal name, date of birth, and contact details
Proof of identification (e.g., passport, driver’s licence)
Emergency contact information
Relevant medical and eye health history, including current medications and allergies
If using insurance, your insurance details and policy number must be provided prior to your appointment.
2. Consent & Privacy
Patients must read and sign our Consent to Treatment Form before any diagnostic or surgical procedure.
Vision Care complies with all Privacy and Health Information laws. Your personal information is kept confidential and will only be shared with authorised health professionals involved in your care.
3. Payment & Financial Arrangements
All consultation fees are payable at the time of booking or before the appointment.
Surgical procedures require full payment before the scheduled surgery date.
We offer financial assistance and payment plan options upon request.
4. Appointment Confirmation
Appointments will only be confirmed once registration is complete and all required documents are received.
If forms are not submitted within the requested timeframe, your appointment may be rescheduled.
5. Changes & Cancellations
Please notify us at least 48 hours in advance if you need to change or cancel your appointment.
Late cancellations or non-attendance may incur a cancellation fee.
6. Updates to Information
It is the patient’s responsibility to update Vision Care with any changes to personal details, medical history, or insurance coverage before future visits.
